Sushi tipping

I've been frequenting sushi restaurants for 15 years now and have always wondered, what is the tipping etiquette?

If it's a place where it's just me and the chef, it's no problem. But what if there is a waiter/waitress in the mix? Do I leave a tip for the waitperson and the chef or do I include everything with the bill or....?
