Ame's Bespoke Dinner

On my previous thread i think 26 said that he did a bespoke, or kaiseki dinner. i just called and the hostess said to just tell the server when you arrive, while 26 said he had to call ahead and arrange things. Just wondering if anyone else has done this. what kind of things were served? how much input did you give? what was cost?
