Olive oil with herbs

How do I make a delicious bottle of "herbed" olive
oil? You know, the kind you pour on your plate at
an Italian restaurant and dip your bread into with
all of those various herbs mixed in?

I know there must be some roasted garlic but what
other herbs? How much of each?
How do you store the finished product and how long
will it last (Shelf life)??
