Not used to gratuity automatically added to my bill...

We had finished our meal at a new NYC area restaurant and went about paying the tab, when I double checked the totals and noticed that the gratuity had already been added to our final bill. We were about to leave another gratuity on top of that. We asked to make sure, and our waiter confirmed the tip was already on the bill. I know this is more common in Europe. Has this happened to you? Would they have said anything if we hadn't noticed? If we had received extra special care, could we leave extra anyway?
