Taxes and tipping

This is a totally silly question, but I want to be prepared and I'm trying to make a budget for my upcoming vacation in Montreal.

1. How is food taxed at restaurants? If something is listed on a menu as $10, does that price include tax, or is the tax added to the check?

2. If the tax is *added* to the price, what is that tax? Is it the standard 7% Federal GST? Is there any Quebec- or Montreal-specific tax on top of that?

3. Is tipping in Montreal similar to tipping in the U.S. (where I am from)? In other words, do I tip on top of the final bill amount? And are the tipping rates the same as in the U.S. (15% for adequate service, 20% for good service)?
