why did my sauce separate?

I've made bananas foster before and it worked fine, so why, using the same recipe, did the butter separate from the brown sugar last night? At first the sugar refused to dissolve (and why did THAT happen?), so I kept whisking and cooking, until it just separated. Then the brown sugar hardened into little chunks.
