Does anyone have info on Takao Ishiyama's sushiya in Tokyo?

He was Kanesaka's apprentice when I ate there in 2012 and I recently saw a couple of reviews for his place. Neither provided any information and I've tried a google search for his name with no luck. The reviews I've seen are:

I'm going to Tokyo for a couple of weeks in Jan/Feb and would love to visit his sushiya while I'm there. If anyone can provide me the address/phone information I would greatly appreciate it.
