Beef roast: covered, not covered, hybrid?

Generic beef pot roast here. 3-5 lbs. Either in a dutch oven or a roasting pan. Some veggies. Do you always cook it closed? Take off the lid at the last hour or 30 min? Do you leave the lid off the whole time?
