Pot au Creme help.

I made this pot au creme recipe from the Julia Child/Jacques Pepin book yesterday. Simple recipe, however when I poured the cream into the chocolate, not only did the chocolate take a long time to melt, but little shards of chocolate remained. This has happened to me before when I made pudding.

What am I doing wrong?
