Mishima--just a bad night or is it in sad decline?

I've always liked Mishima for clean, flavorful food, not to mention that they have my favorite dessert, coffee jello. I went with the elder pup on Sunday though and started with the usually delicious tofu salad. The tofu was FROZEN and the lettuce was sopping and soggy. The server seemed extremely miffed when I pointed it out. I ordered some extra fancy handmade udon which the menu raved about--I'm sorry I forget the name. It came with bland turkey meatballs in bland chicken broth. The noodles were fluffy and inconsequential and I missed the bite of udon. Fortunately, the pup left some of his udon so I got my fix in. The service has declined from brisk to brusque and the portion of coffee jello has declined from ample to dinky. I hate to take Mishima completely off the list 'cause I've enjoyed it so much in the past and it can be really convenient when one is in that part of town with pups. Tell me, PLEASE that it was just an off night...
