Marcella Hazan's Fish Stew?

Has anyone made this? I'd really like to try it, but am a bit intimidated by the fish head requirement. (Not to mention the fact that I don't have a food mill in which to squish said heads). Is it definitely worth getting over myself and buying some fish heads and borrowing mom's food mill? Do you have any substitution suggestions? Ever since I read the recipe I've been sort of fascinated by it! Thanks!
