Japan Trip in September - need help on restaurant suggestions please

My husband and I are going to Tokyo, Kruizawa, Kanazawa, Takayama in Mid September. I have been looking at all the reviews and still, couldn't decide on the restaurants. Last year, we have tried Ishikawa, Den and the Japanese restaurant at Ritz Carlton.

Our budget is about 20K-25K pp before tax and drink. We can stretch to 30K but has to be a phenomenal one.... Attentive service to us is as important as good tasting food. We want an overall enjoyable experience so a little English will help....

So far here's what I have in mind on Tokyo:

Tokyo -
Sushi: Sushisho Saito, Takumi Shingo, Sushi Rinda, Sushisho Masa
Fusion: Florilege, Matsukari, Cork, Hiroo Onogi
Kaiseki: Kohaku...Nogizaka Shin, any other budget friendly ones??
Tempura: Motoyoshi, Takiya

Other than this I have no clue.... I hope someone here reading this post can help me.

By the way, we are also looking for Hibiki 17 (reasonable price at around 16K yen) if anyone of you has seen this please let me know.
