Input Sought on d'Yquem By the Glass

I'd appreciate input on which restaurants serve d'Yquem by the glass, the applicable vintages, the prices and the pour size.

I was very surprised last night to see d'Yquem '91 offered by the glass at Divine Bar, East ($25). While I did not order it, I was told the pour quantity would be about 3-4 oz, which is not bad. While I've never tasted the '91 and it probably could benefit from a few more years, $25 is not a bad price for d'Yquem.

Interestingly, the other place I recall seeing d'Yquem by the glass in NYC recently is at Tuscan (!).

Divine, East was also offering Inniskillin (Riesling) ice wine by the glass (vintage unspecified) at $12-15.
