hot chocolate questions

So I have what may be a really silly question. I want to use the steaming wand from my espresso machine to make hot chocolate. Do I just steam the milk, then just add some quality chocolate to melt in it? Or do I put the chocolate right in the milk and steam it all together?

Also, does milk chocolate work best because of the sugar in it, or am I better off using a bittersweet chocolate and adding sugar?
