Grainy ice cream

Could the problem be the sugar?

I broke up the chocolate (Trader Joe's 72% dark), melted it in half and half, added organic evaporated cane juice sugar (Also Trader Joe's), chilled it, added more half and half, vanilla (homemade), and put it in the machine. Tastes fine, but it's grainy. Any ideas why?
