Where's the Fresh Tofu (Japanese style)?

Where can I find fresh Japanese style tofu in NYC? This is the type that is so creamy (yet firm enough to eat with ohashi) and delectable, many places in Tokyo serve it with salt only. Also served hiyayako style -- with fresh grated ginger (shoga), shoyu (soy sauce) and katsuoboshi (dried bonito flakes).

Any tips on where to find this, either in a restaurant or shop?
