Formosa - North Haven - Okay food, the worst service imaginable....

After my wife and son got back from a week in Paris, we wanted something "easy" last night, so we went to Formosa in North Haven for a late dinner (arriving around 8:45). We've eaten there in the past and enjoyed the experience, but I can state unequivocally that we won't go back. The food was good - albeit served tepid, but the service was absolutely disgraceful.
When we sat down, a waitress came up to us promptly and asked if we wanted anything to drink. My wife and son only wanted water, but I requested a Tsingtao beer. About 3 or 4 minutes later, a second waitress appeared and asked the same question. When I said that another waitress had already taken the order, she said "now tell me." From there. everything went downhill. We had no silverware or napkins (or chopsticks), and had to ask 3 times for them. It took quite a long time to get our entrees (even though the dining room was 80% empty), and when they arrived, one was hot, the other was lukewarm. The third entree arrived several minutes later and this, too, was tepid. In the interim (between asking for silverware and then requesting serving spoons), as the waitress was clearing off another table, she was simultaneously texting (and at that point, we had STILL not received our silverware!!!!!). My son asked for soy sauce, and that never arrived.....
To compound this less than fulfilling experience, a couple (who appeared to be Chinese and regular patrons) sat down in the dining room and the woman promptly began talking loudly on her cell phone, and what made it even worse was that she had it on speakerphone, so from across the dining room, we could hear both her words and that of whoever was on the other end of the phone. After 20 minutes, she was still talking....and when their food arrived, amazingly enough, she continued to talk, holding the phone with one hand and eating with the other!! When we complained to the waitress, she chuckled.....
After receiving the check (again, we had to request it), nobody came back to the table to pick up my credit card. The young man who was hosting was in the dining room and I asked him to please take it. I then told him that this was the worst service we had encountered at a restaurant in a very long time and he, too, chuckled.....
It boggles my mind that management would tolerate a waitperson texting in the middle of the dining room and a patron clearly being annoying to other diners and doing nothing about it.
A truly awful dining experience and a management that is obviously not paying attention.
