How to Use Cast Iron Simmer Plate/Heat Diffuser

Can anyone explain the best way to use these? I have a cast iron one by the same manufacturer as my stove, which the previous home owner's left behind for us when we bought the house. Without the simmer plate, I have found in many circumstances that even on the lowest flame of my smallest burner, I have a lot of trouble maintaining a simmer and instead have more of a rolling boil. I don't usually think ahead enough to preheat the simmer plate, and often times once I've realized I can't hold a low simmer and try to move to the simmer plate, the temperature drops to well below a simmer and has trouble coming back up. How are these meant to be used? Should I be preheating it for several minutes over low heat before moving my already simmering food from another burner?
