2 badly cooked meals, is it me?

2 weeks ago we had dinner at a steakhouse we frequent regularly. While taking my order, something interrupted the waitress, so I repeated that I wanted my steak medium rare. Later the waitress came back, and asked me "you wanted your steak medium, right?" and I said, no, medium rare.

Of course, the steak came medium well, with almost no pink in the middle. Everyone else who had ordered medium rare had steaks that were perfectly done, and the requested medium rare. The waitress asked how the meat was, and I told her mine was overcooked, and I asked the waitress if they could re-do it (she was clearly annoyed at the kitchen, saying "I TOLD them medium rare"). She apologized and said it would be right out. And it was, only this time it was not even rare, but icy cold. Sigh. It was like it was cooked well-done about 1/4 of the way through, then raw in the middle.

She asked how it was, and so I suggested that maybe they could just warm it up a little? Clearly they had rushed to get it out, and hadn't cooked it properly. And by this time I was pretty hungry for my steak. The manager came over, said they'd take it off the bill, but still wanted to give me another steak. I said, take your time, do the steak correctly, and don't take it off the bill. But they insisted. Finally it came out, done correctly, and they also sent us a plate of desserts. I did eat the steak, and wanted to pay, but they wouldn't let us pay for it.

Fast forward a week. Different restaurant. Ordered lamb loin. The waitress asks how to do it, I say medium-rare. I get well-done. So again, I send it back - this time I tell her "don't rush it, we aren't in a hurry, you aren't so busy, tell them to take the time to do it right" and again, it comes out very quickly, and is just raw in the middle. I didn't bother trying to send it back, I just set it aside and didn't eat it. They did take it off the bill with no prompting from me. But the food over all was just not that great, so we won't be back.

But is it me? Is it something I did wrong in ordering? I've never had this trouble before, and suddenly twice in one week's time!
