Just about any Chinese supermarket in the SGV will have it, e.g. 99 Ranch, Hong Kong, Shun Fat, etc.
Some SGV bakeries will also have it, like Cathy's, and some delis will also make it from time to time, like Ding's.

Most dim sum places serve it as a dish, if you go early enough. They usually run out by 10AM or 11AM.
I'm assuming you're asking for hot "Doh Fu Hwa" (tofu flower), with sweet ginger syrup....

"dou fu faa" is Cantonese for "dou fu hua". :)
To the OP -- every 99 Ranch has it. The challenge is finding it. Some places (like the one in the Great Mall of China) keep it near the tofu, some keep it near the Calpico-type drinks, and some (like Anaheim) have it in a random location near the free-standing fish refrigerators -- it'll be a beat-up old metal refrigerated stand and the dou fu faa will be atop it. You're looking for a quart tub wrapped in loose plastic with a small tub of the ginger syrup above it.

My family's favorite place for fresh soy products like the tofu dessert you're looking for is VK foods on Valley in San Gabriel. I'm not a big fan of soy milk but I really like their's.

In Chinatown, you can go into Rainbow Deli in the parking lot of the Ai Hoa Market on Hill St. (enter from College St.) and they'll sell you a pretty large serving for around $2.25.

Like Ipse says, most Chinese supermarkets have it, but it may be hard to locate, as noted by Ubergeek. At Arcadia Supermarket, its on a shelf across from the fish section, at the corner where the pickled veggies are kept.
Also, for linguistic reference, it's "doufu hua" in Mandarin, "doufu faa" in Cantonse, and "dau hwei" in Taiwanese. When I was a kid visiting Taipei, mornings in our neighborhood were punctuated by a tofu vendor crying "Dau hwei! Dau hwei!" as he walked down the alleyways.

if you want something a bit different, i.e. not just with ginger+sugar, one of my faves is the mango tofu soy custard from Phoenix Inn:

It is tofu fa topped with some sago, condensed milk and fresh mangoes.
you can get it at any of the phoenix, either restaurants or their "food boutique".
The resto is on Valley in San Gabriel area, and so is one of their 'food boutiques'
Then there's another food boutique in the same strip mall as Din Tai Fung in Arcadia :)
